We know your time is valuable. For your convenience, we've provided all our patient forms electronically for you to fill out prior to your appointment. Don't have a printer? No problem! Just call us and we can mail the forms to you. And if that still doesn't work for you, just come to your appointment 15 minutes early and we can help you complete the required paperwork.

Q: Do you take new patients?
A: Unfortunately not at this time
Q: What insurance do you accept?
A: We are PPO providers for Aetna, Cigna, Blue Cross Blue Shield, Humana, GEHA and Sunlife. We are a Premier provider for Delta Dental and select MetLife.
Q: Do you have insurance options for retired individuals?
A: Give us a call and chat with Mary. She has some knowledge about individual insurance plans and their pros and cons.
Q: At what age should I start bringing my child to the dentist's office?
A: We recommend a child's first appointment around age 3. As with everything, if you see a problem while brushing before 3 years old, please give us a call. Not all children will cooperate with us at such a young age, but we'll try to work with them, never pushing to inadvertently scare them of the dentist office.
Q: What payment options do you take?
A: Cash, Credit Card (Mastercard, Visa, Discover, American Express), and CareCredit.
Q: What is your appointment cancellation policy?
A: We always send patients a reminder post card and we give you a call the day before. Should you still need to cancel or reschedule, we require at least 24 hours notice. Less than 24 hours notice or no-shows may be subject to a $50 cancellation fee.
Q: What are the hours you are open?
A: Monday-Wednesday 8:00am - 5:00pm, Thursday 7:00am - 3:00pm, Closed for lunch, Closed Friday-Sunday.
